Eyewear: All sales of prescription and non-prescription eyeglasses and sunglasses are final and payment in full is due at time of order, however, patients are welcome to return to the office as many times as needed before the final purchase is made.
Providing Your Own Frames: We will be happy to make prescription lenses if you choose to bring in your own frame(s). Due to the age and wear of an existing frame, they may crack or break when new lenses are inserted, therefore we cannot be responsible for any damages or loss to the frame as they are being used at your request. Annandale Eye Clinic, Cokato Eye Center, Litchfield Eye Center, or our Labs cannot be held responsible for any loss, including during shipment. Regardless of condition or age of the frame, this disclosure applies to any frame(s) being used at your request.
Frames: If your frame(s) break under manufacturer defect guidelines (as determined by our opticians), we will repair or replace them according to manufacturer guidelines from the date of purchase and only if the frame was purchased from Annandale Eye Clinic, Cokato Eye Center, or Litchfield Eye Center. This does not include accidental damage or breakage that has been incurred to the frames. We do not cover loss, theft, or abuse.
Frame and/or Lens Changes: If you change your mind regarding the frame and/or lenses you selected after your custom lenses have been ordered for that frame, you can select a different frame and/or lenses within 60 days from date of purchase under the following conditions: If the frame or lenses chosen are an upgrade, You will owe the upgraded difference between the new and old frame and/or lens price. However, Other changes are made at no charge and no
refund will be given.
Fees for changes may vary depending on your specific insurance plan and the required copays. Please note that some Insurance Companies do not allow changes.
Contact Lenses: With regard to sales of non-specialty soft contact lenses, any unopened & unmarked boxes may be returned for a full refund, or exchanged, within 90 days. However, all sales of specialty gas permeable, hybrid, and scleral contact lenses are final.
